The U.S. Department of State issues U.S. passports, which are required for most international travel. Passports include a variety of personal information about the holder, including name, sex, birthday and place of birth. For these reasons, a lost or stolen passport should be reported and replaced immediately to guard against identity theft. The U.S. Department of State outlines several steps for replacing a lost or stolen passport.
Instructions
1. Report a lost or stolen passport immediately by contacting the U.S. Department of State at 877-487-2778 from 8 a.m. to 10 p.m. ET, Monday through Friday, excluding federal holidays.
2. Complete Form DS-11: Application for a U.S. Passport and Form DS-64: Statement Regarding a Lost or Stolen Passport. These forms can be found online or at any passport agency or acceptance facility.
3. Submit completed forms in person to any passport agency or acceptance facility.
4. Immediately sign your new passport when it's delivered in approximately four to six weeks.
5. Make an appointment at a regional passport agency if the passport is needed in less than two weeks for international travel or if it's needed within four weeks to obtain a foreign visa.
